Continuity of Care
YOUR PREFERRED DOCTOR
All patients are asked to nominate a preferred doctor.
This doctor will maintain your electronic medical record.
This doctor will inform you of periodic health checks as appropriate.
This doctor will also recall you for clinical reasons.
This doctor will do other tasks from time to time e.g. completing medical assessments for pre-employment or insurance applications.
This doctor will be the one whom you see for all routine health care, and for emergency care when possible.
From time to time this doctor will not be able to see you when you wish. The doctors encourage you to see their colleagues at these times.
MANAGEMENT OF YOUR PERSONAL HEALTH INFORMATION
Your medical record is a confidential document.
It is our policy to maintain security of your personal health information at all times and to ensure that it is only available to authorised members of staff.
All records are computerised and password protected.
CHANGING YOUR DOCTOR
Over time your needs may change. You may find one of the other doctors more suitable for your needs and wish to change doctors.
In this case, please discuss this with your new preferred doctor, and if they agree to take over your care, your former doctor will be informed and the name of your new doctor will be recorded on your electronic record.
